Responsibilities

  • Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans.
  • Implement evidence-based physiotherapy techniques, including orthopedics, geriatrics, neuromuscular rehabilitation.
  • Provide therapeutic interventions in the home health environment to optimize patient outcomes.
  • Document patient progress accurately using electronic medical records (EMR systems) and communicate effectively with multidisciplinary teams.
  • Educate patients and caregivers on injury prevention, exercise routines, and self-care strategies to promote long-term health.
  • Stay current with advancements in physiotherapy practices, medical terminology, and anatomy knowledge to deliver high-quality care.

Skills

  • Strong understanding of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
  • Proven experience in discharge planning and care coordination with other disciplines.
  • Skilled in managing diverse patient populations including geriatrics and orthopedic cases.
  • Familiarity with EMR systems for efficient documentation and patient tracking.
  • Excellent patient care skills with the ability to motivate and support individuals through their recovery journeys.
  • Experience in home & community care environments with a focus on home health services.

Pay: $65.00 - $85.00 per hour
