Duties

• Conduct comprehensive assessments of patients’ physical conditions, utilizing your knowledge of physiology and anatomy to develop tailored care plans.
• Educate patients and their families on injury prevention, mobility exercises, and self-care strategies to support long-term wellness.
• Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems while adhering to medical terminology standards.
• Assist in the management of acute care cases by implementing targeted therapies that address immediate needs while considering overall care plans.

Qualifications

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
• Demonstrated experience working in home health environments.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy principles.
• Experience with discharge planning processes and coordinating patient care across multiple settings.
• Familiarity with EMR systems for documentation and patient record management.

Pay: $110.00 - $120.00 per hour
