Physical Therapist

COMPETITIVE SIGN ON BONUS! (Contact for details) Provides physical therapy care, general health guidance and teaching to clients in the home setting and also in rehab department setting (if candidate would so choose) in nursing home (flexible!), practicing within the parameters of the State of Wisconsin for physical therapy, utilizing acceptable principles of practice and following the policies and procedures of HomeCare Health Services. Assumes responsibilities for coordinating care with other health care personnel assisting in the delivery of client care

Responsibilities:

• Develop and implement physical therapy programs that focus on rehabilitation
• Work directly with patients to achieve maximum physical recovery
• Evaluate effects of therapy treatment and communicate patients' progress
• Record and document patient care services
• Collaborate with other team personnel to achieve well-rounded care

Qualifications:

• Previous experience in physical therapy, rehabilitation, or other related field
• Ability to handle physical workload
• Ability to build rapport with patients
• Excellent written and verbal communication skills
• Strong leadership qualities
