We are currently seeking a motivated and compassionate Physical Therapist (PT) to join our growing team.

Position Summary

The Home Health Physical Therapist is responsible for evaluating, planning, and providing skilled physical therapy services to patients in their homes. The Physical Therapist works collaboratively with physicians, nurses, occupational therapists, speech therapists, medical social workers, and home health aides to help patients regain function, improve mobility, reduce pain, prevent falls, and maximize independence.

Responsibilities

  • Perform comprehensive physical therapy evaluations.
  • Develop individualized treatment plans based on physician orders and patient goals.
  • Provide evidence-based therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
  • Perform home safety assessments and recommend appropriate adaptive equipment.
  • Educate patients, caregivers, and family members regarding treatment plans, exercises, fall prevention, and disease management.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Collaborate with physicians and the interdisciplinary care team.
  • Complete timely, accurate documentation in the electronic medical record.
  • Participate in case conferences and quality improvement initiatives.
  • Maintain compliance with Medicare Conditions of Participation, Oregon regulations, and agency policies.

QualificationsRequired

  • Graduate of an accredited Physical Therapy program (DPT or equivalent).
  • Current Oregon Physical Therapist license in good standing.
  • Current CPR/BLS certification.
  • Valid driver’s license, reliable transportation, and automobile insurance.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while managing a flexible schedule.

Preferred

  • Home health experience.
  • Knowledge of OASIS documentation.
  • Experience with Medicare home health regulations.
  • Bilingual candidates are encouraged to apply.

  • Stroke and neurologic rehabilitation
  • General deconditioning
  • Balance disorders and fall prevention
  • Cardiac and pulmonary rehabilitation
  • Dementia and cognitive impairment
  • Chronic disease management

Work Location

Patient homes throughout the Portland Metropolitan Area.
