Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. Responsibilities

• Utilize various therapeutic techniques, including trigger point therapy, to address specific patient needs.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Provide education and support to patients and their families regarding treatment options and home exercises.
• Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
• Document all patient interactions, treatment plans, and progress notes in compliance with medical standards.
• Participate in discharge planning to ensure a smooth transition for patients returning home or moving to other care settings.

Skills

• Proficient in physical therapy practices with a strong foundation in medical terminology.
• Knowledgeable in developing effective care plans tailored for acute care, geriatrics, and home community care settings.
• Strong understanding of physiology and its application in patient rehabilitation.
• Excellent communication skills for effective interaction with patients, families, and interdisciplinary teams.
• Ability to demonstrate empathy and patience while working with diverse patient populations.
• Familiarity with discharge planning processes to facilitate successful patient transitions.
