We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our outpatient care team—providing therapy directly in patients' homes. Responsibilities

• Assist Physical Therapists in carrying out personalized outpatient treatment plans in patients’ homes.
• Perform therapeutic exercises, mobility training, and other rehab techniques as directed.
• Track and document patient progress accurately using digital or written records.
• Instruct patients and caregivers on exercises, mobility strategies, and safety practices.
• Ensure a safe and professional environment during each home visit.
• Communicate with PTs and healthcare team members to support high-quality, coordinated care.
• Treat a variety of patients, including those recovering from surgery, auto accidents, or managing chronic conditions.
• Apply techniques like trigger point therapy, neuromuscular re-education, and manual therapy to improve outcomes.

Requirements

• Associate’s degree from an accredited Physical Therapist Assistant program.
• Current state licensure or eligibility as a PTA.
• Knowledge of outpatient rehab techniques, anatomy, and medical terminology.
• Experience with home-based outpatient care or similar settings preferred.
• Ability to manage time independently and deliver care across a geographic area.
• Strong interpersonal and communication skills.
• Passion for helping patients regain independence and improve function at home.
