Physical Therapist Assistant - Home Health Visits - San Fernando Valley South

Home Health Links is building PTA coverage in San Fernando Valley South. This position is intended for therapists who want flexible home health visit volume, local routes, and one-on-one care with older adults in the home setting.

Coverage area: Reseda, Tarzana, Woodland Hills, Canoga Park, West Hills, Encino, North Hollywood, Panorama City, Sherman Oaks, Studio City, Toluca Lake, Topanga, Universal City, Valley Village, Van Nuys, Winnetka

Schedule: Weekdays, weekends, or both

Visit volume: Part-time visit volume

Responsibilities

• Complete appropriate home health visits
• Communicate schedule and patient updates
• Complete documentation within expected timelines
• Coordinate with HHL support when questions or scheduling issues come up.

Requirements

• Active California license for the listed discipline
• Reliable transportation
• Ability to document on your own device
• Comfort communicating with patients, families, agencies, and the HHL team.
