Physical Therapist - Clarence Location

Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our team at our Clarence location. The ideal candidate will possess a strong foundation in physiotherapy, with expertise in patient care across diverse settings including inpatient, outpatient, home health, and community care. This role offers an exciting opportunity to deliver comprehensive rehabilitation services, develop personalized care plans, and improve patient outcomes through evidence-based practices. Duties

  • Assess patients’ physical conditions through detailed evaluations, including mobility, strength, and functional capacity.
  • Develop and implement individualized care plans tailored to each patient’s specific needs and recovery goals.
  • Provide therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote healing and restore function.
  • Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital or clinical settings to home or community environments.
  • Document patient progress accurately using electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
  • Educate patients and their families on physiotherapy techniques, injury prevention, and ongoing self-care strategies.
  • Collaborate with healthcare professionals across specialties—including orthopedics, geriatrics, pediatrics, and acute care—to optimize treatment outcomes.
  • Participate in hospital or clinic team meetings to review patient cases, update treatment protocols, and contribute to quality improvement initiatives.

Skills

  • Extensive knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
  • Proven experience in discharge planning and managing care across inpatient, outpatient, home health, and community settings.
  • Strong background in acute care environments such as hospitals or clinics with familiarity in inpatient protocols.
  • Experience working with diverse populations including pediatrics, geriatrics, orthopedics, and individuals requiring home or community-based care.
  • Proficiency in EMR systems for documentation and patient record management.
  • Skilled in various physiotherapy techniques including trigger point therapy and orthopedic rehabilitation methods.
