Physical Therapist - For Home health agency

Overview

We are seeking a licensed and compassionate Physical Therapist (PT) to provide skilled therapy services in patients’ homes in Key West. This position requires traveling to patient residences to perform physician-ordered therapy services and deliver individualized, goal-oriented care.

The ideal candidate is experienced in home health, comfortable working independently, and proficient in documentation using Kinnser/WellSky EMR.

Responsibilities

• Travel to patients’ homes to provide ordered physical therapy services
• Perform comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Complete OASIS assessments when required
• Provide therapeutic exercises, gait training, balance training, strengthening, and mobility interventions
• Educate patients and caregivers on home exercise programs and fall prevention
• Monitor patient progress and adjust treatment plans as needed
• Communicate with physicians and interdisciplinary team members regarding patient status
• Accurately document evaluations and visits in Kinnser/WellSky EMR system
• Maintain compliance with HIPAA and infection control standards

Qualifications

• Active Florida Physical Therapist (PT) license
• Home health experience preferred
• OASIS experience preferred
• Experience with Kinnser/WellSky EMR strongly preferred
• Strong clinical assessment and documentation skills
• Ability to work independently in patient homes

Pay: $70.00 - $90.00 per hour
