PHYSICAL THERAPIST HFM, NEMS - SHEBOYGAN NORTH REHAB

Evaluates and treats all referred Physical Therapy patients according to physician orders. Home Care: Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.

Degree in Physical Therapy required.

Wisconsin licensed Physical Therapist or eligible required. If eligible, must obtain Wisconsin license within 3 months of hire. CPR certification is required.

At least one year experience in a medical setting preferred. Good communication skills required.
