Physical Therapist, Home Health

Starting Rates:

• Oasis Start of Care $100 per visit
• Oasis Recertification/Resumption of Care/Evaluation $85 per visit
• Routine Visit $75 per visit
• Mileage pay

Elite Home Rehab and Healthcare is an accredited ACHC skilled home health agency servicing Central Indiana. We are a clinician owned agency, dedicated to providing quality care to our patients as well as helping our clinicians grow professionally. We are looking for a Part or Full Time Physical Therapist for Indianapolis, IN and surrounding areas.

Duties of Position:

Provide skilled physical therapy services on an intermittent basis to patients in their home in accordance with the physician’s orders.

Position Responsibilities:

• Evaluate the patient’s level of functioning and recommends mechanisms to enhance the patient’s hearing ability.
• Provided initial and ongoing comprehensive assessments of the patient’s needs in including Outcome and Assessment Information Set (OASIS) assessments.
• Establish a Physical Therapy treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy.
• Teach, supervise and counsel the family and patient in the total Physical Therapy program and other related problems of the patient at home.
• Record on a timely basis all evaluation data, treatments, and patient’s reaction to treatment and/or any changes in the patient’s condition.
• Communicate with the physician at least every 30 days or whenever changes occur regarding the patient’s status and obtains additional orders, if necessary.
• Participate in agency activities, case conferences, and meetings, when appropriate.
• Participate with staff, patients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary.

Qualifications:

• Must have the ability to drive and access patients’ homes.
• Licensed in the State of Indiana to practice Physical Therapy.
• Preferred to have at least one year of institutional experience.
• Must have a criminal background check.
• Must have a current CPR certification.
