Best Choice Home Care is looking for an enthusiastic, passionate, and skilled Physical Therapist to join our team of home care professionals. We have multiple positions available. Part-time, full-time, and/or PRN. We offer very flexible scheduling and we’re always looking for the most professional, qualified individuals in the communities we serve.

Physical Therapist will be mainly responsible for completing Start of Care and PTA's Supervision

Compensation: Based on visits, Re-certs. evals, DC and Start of care $70- $130

We are expanding! Currently hiring in Marion, Rush, Shelby, Hendricks, Hamilton, Boone, Clack, Handcock, Johnson, Morgan, Brown and Madison Counties.

Physical Therapists are responsible for administering therapy to patients on an intermittent basis in their place of residence. Home Health is the perfect setting for the PT that loves the utilization of the patient’s individual environment to improve the best outcome for each home bound patient.

Job Responsibilities:

- Providing physician-ordered physical therapy

- Supervising a PTA (PTs only)

- Returning the individual to optimum functional levels within the patient's capabilities

- Participating with other health care personnel within the interdisciplinary team

- Actively participate in patient care planning and treatment

- Directing and supervising personnel as required

- Taking initial history and making initial evaluations

- Consulting with and writing reports to physicians regarding any patient changes, as well as overall progress

- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition

- Instructing patients and family/significant others in home programs and activities of daily living
