Physical Therapist, Home Health

Siesta Key Healthcare Services LLC is hiring a licensed Physical Therapist (PT) to provide skilled rehab services to help clients regain strength, mobility, and independence in the comfort of their homes. New grads welcome!

Key Responsibilities:

What You’ll Do:

• Perform in-home physical therapy evaluations and treatments
• Create and update individualized care plans in Kiinser
• Educate patients and caregivers on exercises and safety
• Document visits and progress in EMR
• Coordinate care with nurses and the clinical team

What You’ll Bring:

• Florida PT license and reliable transportation
• Strong clinical judgment and communication skills
• Home health experience a plus (not required)

Compensation:

• $50 per completed home health visit
• Flexible schedule + mileage reimbursement
• Create and update individualized care plans
• Educate patients and caregivers on exercises and safety
• Document visits and progress in EMR
• Coordinate care with nurses and the clinical team
