Job Responsibilities: This is home health positions for a Physical Therapist (PT) with multiple openings.

The PT is responsible for developing the Plan of Care and implementing interventions based on the POC and goals. PT will be responsible for maintaining contact with the PTA and coordinating care between patient and POC. The therapist will manage their own schedule while supporting a specific or multiple ALF communities or drive to patients throughout the day. This job is very autonomous, perfect for those who don’t enjoy being kept working in a facility all day.

Locations/ Counties served: Collier, Lee, Polk, Manatee, Hillsborough, Sarasota, Duval, Broward, Flagler, and expanding to other areas throughout Florida.

Qualifications: Current Florida State License as a Physical Therapist or Assistant, Current Florida Driver’s License, registration, and auto insurance. Current CPR, physical and PPD. Preferred experience in home health. Good organizational and communication skills.

Responsibilities:
- Evaluate and assess patients' physical conditions and develop appropriate treatment plans
- Provide hands-on therapy to patients using a variety of techniques, including trigger point therapy
- Educate patients and their families on exercises and techniques to improve mobility and manage pain
- Monitor and document patients' progress, adjusting treatment plans as necessary
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Stay up-to-date with advancements in the field of physical therapy and incorporate new techniques into practice

Requirements:
- Valid license to practice as a Physical Therapist (Assistant)
- Knowledge of medical terminology, anatomy, and physiology
- Strong interpersonal skills and ability to effectively communicate with patients, families, and healthcare team members
- Excellent problem-solving skills and ability to adapt treatment plans based on individual patient needs

Pay: $100,000.00 - $150,000.00 per year
