Home Health Links is building PT coverage in San Fernando Valley North. This position is intended for therapists who want flexible home health visit volume, local routes, and one-on-one care with older adults in the home setting.

Coverage area: Mission Hills, Northridge, Chatsworth, Sylmar, North Hills, Granada Hills, Porter Ranch, Panorama City, Sun Valley, Pacoima

Schedule: Weekdays, weekends, or both

Visit volume: Part-time visit volume

Responsibilities

• Complete appropriate home health visits
• Communicate schedule and patient updates
• Complete documentation within expected timelines
• Coordinate with HHL support when questions or scheduling issues come up.

Requirements

• Active California license for the listed discipline
• Reliable transportation
• Ability to document on your own device
• Comfort communicating with patients, families, agencies, and the HHL team.
