Duties
- Assess patients’ physical conditions through detailed evaluations, including physiology and anatomy knowledge, to develop effective care plans.
- Implement customized physiotherapy interventions such as trigger point therapy, orthopedics techniques, and geriatrics-focused exercises to facilitate recovery.
- Conduct discharge planning and coordinate with healthcare teams to ensure seamless transition from inpatient or outpatient settings to home care.
- Deliver targeted therapy sessions for diverse populations, geriatrics, and individuals recovering from surgery or injury.
- Educate patients and caregivers on proper care techniques, injury prevention strategies, and the use of assistive devices within home and community settings.
- Maintain accurate documentation of patient progress using EMR (Electronic Medical Records) systems and ensure compliance with medical terminology standards.
- Collaborate with multidisciplinary teams to adjust care plans based on patient progress and changing needs in home health environments.
Requirements
- Valid licensure as a licensed Physical Therapist with current registration to practice in your region.
- Proven experience in home & community care, outpatient clinics, or hospital inpatient settings with a strong understanding of acute care principles.
- Demonstrated expertise in physiotherapy techniques including trigger point therapy, orthopedics, and geriatrics.
- Knowledge of medical terminology, anatomy, physiology, and discharge planning processes.
- Prior experience working with EMR systems for documentation and patient management.
- Excellent patient care skills with the ability to communicate effectively with diverse populations including pediatrics and seniors.
- Familiarity with home health protocols, outpatient services, hospital experience, and clinic environments is highly desirable.
