Physical Therapist | Home Visits – Grand Rapids | Full Time & Part Time

Atrio Home Care, a trusted partnership between Holland Home and Resthaven, is seeking a compassionate and motivated Physical Therapist to join our Grand Rapids team, serving clients within a 35 mile radius. This role offers the perfect blend of autonomy, flexibility, and purpose—ideal for someone who values personalized care and work-life balance. We are currently have full and part time opportunities available.
What You’ll Do:
- Provide one-on-one in-home physical therapy services
- Conduct evaluations and develop individualized treatment plans
- Collaborate with physicians and interdisciplinary team members
- Help clients regain mobility, strength, and independence
- Document progress and support discharge planning
- Educate clients, caregivers and families, on therapy goals and techniques
Schedule & Location:
- Territory: Kent County
- Days: Full & Part-time positions available
- Shift: Day shift (8 AM – 5 PM)
- Weekend Rotation: Every 7-8 weekends, no on-call
Compensation & Perks:

What You Bring:
- Doctorate in Physical Therapy (required)
- Michigan PT license (required)
- 1+ year of PT experience (home health preferred)
- Geriatric experience highly valued
- Reliable transportation, valid driver’s license, and auto insurance
