Position

As a Physical Therapist (PT) for Ageless Simplicity Services, you will be responsible for providing quality patient care in accordance with current guidelines and procedures. You will be responsible for the care of the patients you treat and the well-being of the patient in your care. We are looking for a dedicated individual who is passionate about helping others.

Responsibilities:

1. Comprehensive Patient Assessments:

• Conduct thorough assessments of patients’ physical conditions, functional abilities, and mobility in their home environments.

2. Individualized Treatment Plans:

• Develop personalized and evidence-based treatment plans tailored to each patient’s specific needs and goals.

3. Therapeutic Interventions:

• Provide hands-on therapeutic interventions to improve patients’ strength, flexibility, balance, and overall mobility.

4. Home Exercise Programs:

• Design and instruct patients in home exercise programs to enhance their rehabilitation progress between visits.

5. Adaptive Equipment Recommendation:

• Assess the need for and recommend appropriate adaptive equipment and assistive devices to improve patients’ independence and safety.

6. Pain Management:

• Implement effective pain management strategies through therapeutic exercises, modalities, and patient education.

7. Fall Prevention Strategies:

• Evaluate and address fall risks within the home environment, implementing preventive measures to enhance patient safety.

8. Collaboration with Care Team:

• Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated and comprehensive patient care.

9. Patient and Family Education:

• Educate patients and their families on therapeutic exercises, preventive measures, and strategies to manage and improve their physical conditions.

10. Progress Monitoring and Documentation:

• Regularly monitor and document patient progress, adjusting treatment plans as needed to achieve established goals.

11. Telehealth Services:

• Adapt services to include telehealth when necessary, ensuring continued support and monitoring for patients remotely.

12. Adherence to Ethical and Legal Standards:

• Adhere to ethical and legal standards in physical therapy practice,⬤
• Other duties as assigned.

Skills
Proven working experience as a physical therapist.

• Excellent knowledge of medical procedures and treatments (
• Good understanding of health & safety regulations and best practices.
• Good communication skills (verbal & written).
• Friendly attitude with a customer-oriented approach.
• Computer literacy with experience in EMR (EHR) system.
• High school diploma or equivalent; Bachelor’s degree preferred.
