As a Home Health Physical Therapist with Home Nursing With Heart, you will be responsible for assessing patients' physical abilities, developing individualized treatment plans, and implementing therapeutic interventions to promote mobility, reduce pain, and improve overall quality of life. You will work closely with patients, their families, and a multidisciplinary team to ensure that each individual receives the highest level of care and support.

Job Responsibilities:

• Provide patient-centered care to individuals in their homes
• Conduct assessments to develop personalized treatment plans
• Implement therapeutic exercises and techniques to improve mobility and function
• Educate patients and caregivers on proper techniques and exercises
• Monitor progress and adjust treatment plans as needed

Qualifications:

• Current Physical Therapy license in the state of Nebraska
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered approach to care
• Current CPR/BLS Certification
