At Visiting Nurse Service and Hospice of Suffolk (VNSHS), our rehabilitation team delivers high-quality, patient-centered care that truly makes a difference. As a Physical Therapist (Per Visit), you’ll work autonomously while collaborating with an interdisciplinary team to support patient recovery and wellness.

Your Impact:

The Physical Therapist provides skilled rehabilitative services and case management oversight to home care patients, helping them achieve functional goals and improve quality of life.

You Will:

Perform initial evaluations and ongoing physical therapy assessments

Develop and implement patient-centered plans of care

Provide skilled therapy interventions to improve mobility, function, and strength

Utilize telehealth when appropriate (including pulmonary and cardiac rehab support)

Collaborate with physicians and the interdisciplinary care team

Track patient progress and adjust treatment plans as needed

Maintain accurate, timely documentation (including OASIS)

Participate in care management and team communication

Complete required patient notifications and compliance documentation

What You Bring:

New York State Physical Therapist License (required)

Current NYS registration

Previous experience as a Physical Therapist

Strong clinical, documentation, and communication skills

Ability to work independently in a home care setting
