Physical Therapist (PT) provides skilled physical therapy services to patients in their homes, helping them improve mobility, reduce pain, restore function, and maintain independence. The PT develops individualized care plans and collaborates with interdisciplinary healthcare teams to ensure optimal patient outcomes.

Responsibilities for Physical Therapist (PT)

  • Perform initial evaluations and re-evals of patients in the home setting
  • Develop and implement individualized treatment plans based on patient needs and physician orders
  • Provide therapeutic exercises, manual therapy, gait training, balance training, and functional mobility training
  • Educate patients and caregivers on home exercise programs, safety, and fall prevention
  • Accurately document patient progress, treatments, and outcomes in compliance with regulations
  • Coordinate care with physicians, nurses, occupational therapists, and other team members
  • Modify treatment plans based on patient progress and clinical judgment
  • Ensure compliance with Medicare/insurance guidelines and agency policies

Physical Therapist Assistant (PTA) works under the supervision of a licensed Physical Therapist to deliver prescribed physical therapy treatments in the home setting. The PTA supports patients in achieving improved mobility, strength, and functional independence.

Responsibilities for Physical Therapist Assistant (PTA)

  • Provide physical therapy treatments as directed by the supervising PT
  • Assist patients with therapeutic exercises, mobility training, and activities of daily living
  • Monitor and report patient progress and changes in condition to the PT
  • Educate patients and caregivers on exercises and safety techniques
  • Accurately document treatments and patient responses
  • Maintain compliance with all regulatory and agency guidelines
  • Communicate effectively with the interdisciplinary care team

Qualifications:

  • Active Florida state license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
  • Valid driver’s license and reliable transportation
  • Ability to meet same-day documentation standards
  • Home health experience a plus (training provided)
  • Strong clinical assessment and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage time effectively

Pay: $60,000.00 - $160,000.00 per year

Work Location: Hybrid remote in Vero Beach, FL 32966
