South Mountain Post Acute is a 5-Star Skilled Nursing Facility in South Phoenix seeking a Licensed Physical Therapist to join our in-house therapy team.

What you’ll do:

• Evaluate patients within 48 hours of referral
• Develop and implement individualized treatment plans
• Supervise PTAs and rehab aides
• Collaborate with interdisciplinary care teams
• Participate in care conferences and discharge planning
• Document treatments and progress in EMR
• Coordinate adaptive and durable medical equipment

Qualifications:

• Active Physical Therapist license
• All experience levels welcome , new grads encouraged
