Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function. You will work closely with an interdisciplinary team to deliver personalized, patient-centered care.

Key Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Provide therapy to improve mobility, strength, balance, and functional independence
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and timely clinical documentation
• Communicate effectively with physicians, residents, families, and care team members
• Participate in care planning and discharge planning
• Conduct staff education and in-service training
• Supervise PTAs, aides, and students as needed

Qualifications

• Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
• Active, unencumbered Physical Therapist license in South Carolina (required)
• Experience in a skilled nursing or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong clinical, communication, and leadership skills
• Commitment to patient-centered care
