Job Overview
We are seeking a dedicated and skilled Physical Therapist PRN (as needed) to provide exceptional home health care services. The ideal candidate will possess a strong foundation in physiotherapy, with experience in diverse clinical settings including inpatient, outpatient, hospital, and community care. This role offers the flexibility to deliver patient-centered therapy in the comfort of patients' homes, promoting recovery and independence through tailored treatment plans. Duties
- Conduct comprehensive assessments of patients’ physical abilities and develop individualized care plans aligned with medical and therapeutic goals.
- Deliver physiotherapy interventions including manual therapy, trigger point therapy, and exercise programs to improve mobility, strength, and function.
- Manage discharge planning processes to ensure seamless transition from hospital or inpatient settings to home or community environments.
- Collaborate with interdisciplinary teams to coordinate patient care, ensuring adherence to treatment protocols and safety standards.
- Maintain accurate documentation of patient progress using electronic medical record (EMR) systems, ensuring compliance with healthcare regulations.
- Provide specialized care for pediatric, geriatric, orthopedic, and neurological patients within home health settings.
- Educate patients and caregivers on injury prevention, post-treatment exercises, and health maintenance strategies.
- Stay current with advancements in physiotherapy techniques and integrate evidence-based practices into patient care.
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
- Proven experience in discharge planning and managing care across acute care, inpatient, outpatient, and community settings.
- Expertise in developing comprehensive care plans tailored to individual patient needs.
- Familiarity with EMR systems for documentation and patient management.
- Excellent patient care skills with the ability to communicate effectively across diverse populations including pediatrics and geriatrics.
- Experience with home & community care environments is highly desirable.
