Revital Rehab Services is seeking a licensed Physical Therapist (PT) to provide full-time care at our partner facility, Cedar Haven Healthcare Center in Lebanon, PA. This role offers a meaningful opportunity to help residents regain strength, mobility, and independence in a supportive and collaborative environment.

What You’ll Do:

• Evaluate patients and develop personalized physical therapy treatment plans

• Deliver evidence-based, patient-centered interventions

• Monitor progress, update goals, and collaborate closely with the interdisciplinary team

• Ensure all services meet Revital’s high standards for quality, documentation, and ethical care

Qualifications:

• Licensed Physical Therapist (PT) in Pennsylvania

• Current BLS certification

• Strong clinical judgment, communication skills, and a commitment to patient advocacy
