We are seeking a compassionate and highly skilled Physical Therapist (PT) to join our rehabilitation team within a long-term care and skilled nursing facility. The Physical Therapist is responsible for evaluating residents’ functional needs, developing individualized treatment plans, and delivering high-quality therapeutic interventions that enhance mobility, independence, safety, and overall quality of life.

Conduct comprehensive physical therapy evaluations for short-term rehab and long-term care residents.

Develop individualized treatment plans based on clinical findings, resident goals, and interdisciplinary input.

Provide evidence-based therapeutic interventions, including strengthening, balance training, mobility training, gait training, and pain management techniques.

Instruct residents in exercises, functional mobility skills, safe ambulation, and use of assistive devices.

Collaborate closely with nursing, occupational therapy, speech therapy, and social services to ensure coordinated resident care.

Document assessments, treatments, progress notes, and discharge summaries in accordance with state, federal, and facility requirements.

Participate in care plan meetings and provide clinical recommendations to optimize outcomes.

Educate residents, families, and caregivers on ongoing therapy needs, home exercises, and safety considerations.

Monitor resident progress and adjust treatment plans as clinically appropriate.

Ensure compliance with all regulatory and professional standards, including HIPAA and CMS guidelines.

Maintain a positive, resident-centered approach with a focus on dignity, respect, and quality of life.

Current and valid Physical Therapist (PT) license in the state of practice.

Graduate of an accredited Physical Therapy program.

Previous experience in skilled nursing, geriatric care, or post-acute rehabilitation preferred.

Strong clinical reasoning, assessment, and therapeutic skills.

Excellent communication and teamwork abilities.

Ability to safely lift, transfer, and support residents as needed.
