Physical Therapist (PT) – Home Care Per Diem / Contract | Flexible Schedule | Immediate Openings

We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients in the community. This is a great opportunity for a therapist looking for flexible scheduling, consistent referrals, and a supportive agency team.

Responsibilities:

• Provide skilled physical therapy services in the home setting

• Complete patient evaluations and develop individualized treatment plans

• Deliver therapeutic exercises, gait training, balance training, and mobility support

• Educate patients and families on home exercise programs and safety

• Complete accurate and timely documentation in compliance with agency standards

• Communicate progress and updates with the clinical team as needed

Qualifications:

• Active New Jersey Physical Therapist license (required)

• Strong communication and interpersonal skills

• Reliable transportation

• Ability to work independently and professionally
