Position Summary

The Physical Therapist (PT) provides skilled physical therapy services to patients in the home health setting. This role focuses on evaluating patient needs, developing individualized treatment plans, and helping patients improve mobility, strength, and overall functional independence.

Key Responsibilities

  • Perform initial evaluations and ongoing reassessments of patients

  • Develop and implement personalized treatment plans based on patient goals

  • Provide therapeutic exercises, manual therapy, and functional training

  • Educate patients and caregivers on home exercise programs and safety

  • Collaborate with the interdisciplinary care team to ensure optimal outcomes

  • Monitor patient progress and adjust care plans as needed

  • Complete timely and accurate documentation in compliance with Medicare and company standards

  • Participate in case conferences and team meetings as appropriate

  • Current Florida Physical Therapist license

  • 1 year of clinical experience (home health preferred)

  • Experience with WellSky (Kinnser) EMR preferred

  • Experience completing OASIS assessments preferred

  • Strong clinical assessment and problem-solving skills

  • Excellent communication and patient care skills

  • Ability to work independently and manage time effectively

  • Valid driver’s license and reliable transportation
