Outpatient Physical Therapist Job – Mesa, AZ

Citadel Outpatient Therapy is hiring a licensed Physical Therapist (PT) to join our growing outpatient therapy team in Mesa, Arizona.

This is a unique mobile outpatient position where you’ll provide therapy services within partnered Assisted Living communities. You’ll treat patients in a community-based outpatient setting while working alongside a collaborative, in-house rehab team.

Physical Therapist (PT) Responsibilities

• Perform physical therapy evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide evidence-based outpatient physical therapy services
• Treat orthopedic, neurological, and geriatric conditions
• Improve strength, balance, mobility, and functional independence
• Document patient progress in EMR
• Collaborate with therapy, nursing, and care teams
• Educate patients and families on home exercise programs

Physical Therapist (PT) Requirements

• Active Arizona Physical Therapist License
• DPT or PT degree from accredited program
• Current CPR Certification
• Arizona Fingerprint Clearance Card
• Outpatient or geriatric experience preferred
• New grad PTs encouraged to apply
