Job Title: Physical Therapist (PT) – PRN / Per Diem – Home HealthJob Summary:
We are seeking a skilled and compassionate Physical Therapist (PT) on a PRN/per diem basis to provide in-home therapy services to patients. The PT will evaluate, develop, and implement individualized treatment plans to improve mobility, strength, balance, and overall functional independence while helping prevent hospitalizations and promote safe recovery in the home setting.
Key Responsibilities:
- Perform initial evaluations and ongoing reassessments of patients in the home setting
- Develop and implement individualized treatment plans based on physician orders and patient needs
- Provide therapeutic exercises, gait training, balance training, and mobility interventions
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Assess need for assistive devices (walker, cane, wheelchair) and provide training on proper use
- Monitor patient progress and adjust treatment plans as needed
- Document all visits accurately and timely in accordance with agency, state, and Medicare guidelines
- Communicate patient status and progress with physicians and interdisciplinary team members
- Participate in care coordination and case conferences as needed
- Ensure compliance with infection control and safety standards
Qualifications:
- Active California Physical Therapist (PT) license in good standing
- Current CPR/BLS certification
- Minimum 1 year of physical therapy experience (home health preferred)
- Strong knowledge of rehabilitation techniques and functional mobility training
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel to patient homes within assigned territory
- Strong clinical, communication, and documentation skills

Physical Requirements:
- Ability to stand, walk, bend, and assist patients with mobility
- Ability to lift up to 50 lbs (as needed for patient transfers)
Schedule Type:
- PRN / Per Diem (as needed based on patient census)
Pay: $100.00 - $105.00 per hour
