Job Description: Physical Therapist (PT), Occupational Therapist (OT)

The Opportunity

As a Therapist with Philadelphia Home Health Care, you will play a pivotal role in our mission to provide elite patient care across Central Florida. With our recent leadership transition, we are focused on streamlining communication and providing our team with direct access to clinical leadership to ensure you feel confident and supported in the field.

Key Responsibilities

  • Comprehensive Assessment: Conduct thorough physical and psychosocial assessments of patient needs in their home or facility environment.
  • Care Planning: Develop, implement, and evolve individualized care plans in coordination with a multidisciplinary team.
  • Clinical Intervention: Deliver high-quality therapy interventions, including therapeutic exercises, activities, functional independence, and patient education.
  • Collaboration: Act as a key point of contact between patients, families, and our new leadership team to ensure seamless care delivery.
  • Documentation: Maintain accurate, timely clinical records in compliance with state and federal regulations.

Qualifications

  • Active, unencumbered PT/OT License in the state of Florida.
  • Proven commitment to high-quality patient care and clinical excellence.
  • Strong communication skills and the ability to work effectively within a multidisciplinary team.
  • Self-motivated with excellent time-management skills (ideal for those making their own schedules).
  • Reliable transportation to cover zip codes within your chosen area.

Pay: $65.00 - $100.00 per hour
