Physical Therapist/Physical Therapist Assistant Macomb County

Help our home health patients get physical. A Choice Home Health Care is looking for a qualified contingent Physical Therapist or Physical Therapist Assistant to provide valuable services to Medicare patients in their place of residence. This exciting opportunity will give you the chance to travel to clients in Macomb County and surrounding areas.

Duties:

· Provide and direct physical therapy services to patients including assessment, treatment, program planning and implementation

· Optimize patients’ physical abilities and skills through therapy, including evaluating, treating, participating in the care planning process, and documenting progress

· Establish patient schedules, develop treatment plans, and identify long and short-term goals and methods to achieve them

· Coordinate treatment plans with patient, family and other team members, then implement treatment and document patient’s response

· Provide adaptive devices, orthotics or prosthetics and train patient, family and appropriate staff in their use

· Review the quality and appropriateness of the services delivered and of individual physical therapy programs for effectiveness and efficiency

· Participate in patient, family and staff education, conferring formally and informally with other team members and attending training and facility meetings

· Comply with all healthcare professional standards, participate in Continuous Quality Improvement Programs, and adhere to confidentiality standards

· Will provide H 1 B Visa Sponsorship

Job Requirements:

· Bachelor of Science in Physical Therapy from an accredited program

· Completion of the National Certification Examination for Registered Physical Therapist with a license to practice physical therapy in Michigan

· Ability to work varying hours, including weekends and holidays

· Basic Microsoft Word and Excel skills

· OASIS knowledge (required) and EMR experience (preferred)

· Ability to stand, sit, lift 50 lbs, and perform other basic physical movements

· Reliable transportation with a Michigan Driver’s License and proof of auto insurance
