Physical Therapist/Physical Therapists Assistants (PT/PTA) – Home Health Services

We’re seeking a Physical Therapist/Physical Therapists Assistants (PT/PTA) to provide personalized home-based therapy that focuses on restoring function, improving safety, and enhancing quality of life.

• Conduct initial evaluations and develop individualized care plans based on patient needs.
• Provide therapeutic interventions focused on improving daily living skills, mobility, and safety in the home environment.
• Educate patients and caregivers on home exercise programs, adaptive equipment, and strategies to enhance independence.
• Document all visits, progress notes, and care updates accurately and in compliance with home health regulations.
• Collaborate with nurses, and other disciplines to ensure coordinated and effective care.
• Maintain accurate documentation in compliance with agency and regulatory requirements.
• Monitor progress and adjust care plans as needed to achieve optimal outcomes.

Requirements/Qualifications:

• Physical Therapist/Physical therapist Assistant License
• Active Driver's License
• CPR Certification
• Liability Insurance
• Auto Insurance
• Experience in Home Health (Preferred)

This is an Independent Contractor position, AZ Business License preferred.

• Day shift
• Monday to Friday
• On call
• Weekends as needed
