Physical therapy assistant — private household

About the role
A private family on the Upper East Side is seeking a compassionate and skilled physical therapy assistant to provide in-home support. The primary focus is guiding and supervising adherence to a prescribed PT program for a family member managing multiple complex medical conditions. This is a relationship-driven role requiring patience, discretion, and genuine clinical competence in a home environment rather than a clinical setting.
The core schedule is 9 hours per week (3 hours, three days), but the right candidate will have the opportunity to expand their hours in a broader caregiving capacity — assisting with daily activities, medical appointments, and general support for a family navigating complex health challenges. Additional hours and scope are flexible and mutually negotiated.
Responsibilities
— Guide the patient through a prescribed physical therapy program, ensuring correct form, appropriate intensity, and safe progression
— Monitor and document session progress, symptom changes, and any concerns for communication back to the treating physical therapist
— Adapt session pacing to account for variable symptom days, including fatigue, pain flares, or autonomic symptoms, while keeping the program on track
— Educate and reinforce proper movement mechanics, posture, and home exercise technique between formal sessions
— Maintain clear, professional communication with the family regarding observations and any red flags warranting clinical follow-up
— Optionally: provide broader caregiving support on additional days or hours, including accompanying the patient to medical appointments, assisting with daily activities, and offering general household health-support presence Requirements
— Associate's degree in physical therapy assisting or related field
— Current New York State PTA licensure or certification in good standing
— Experience working with patients who have complex or multi-system medical presentations
— Strong interpersonal skills and comfort operating independently in a private home setting — Professionalism, discretion, and a warm, patient-centered approach Preferred experience (not required) + Ehlers-Danlos syndrome (EDS) or hypermobility spectrum disorders
— understanding of joint instability, load management, and stabilization-first progressions + Femoral and/or acetabular hip retroversion and/or FAI
— familiarity with range-of-motion modifications and impingement-aware movement patterns + Postural orthostatic tachycardia syndrome (POTS) or dysautonomia
— understanding of position-change protocols, orthostatic precautions, and recumbent exercise modifications + Prior experience in a private home, concierge, or caregiving role alongside clinical work
Pay: $40.00 per hour
