Northern Counties Health Care is seeking a compassionate and experienced Physician to join our dedicated team at Saint Johnsbury Community Health Center. This full-time role offers the opportunity to provide high-quality care to a diverse rural population as part of a collaborative, mission-driven team.

Responsibilities:

• Deliver comprehensive primary care, emphasizing health promotion, continuity, and disease prevention.

• Maintain detailed and accurate medical records through EMR, including assessments, treatment plans, and follow-up care.

• Order and interpret diagnostic tests in alignment with clinic protocols.

• Provide after-hours on-call coverage and hospital rounds as part of a shared schedule.

• Refer patients to appropriate specialists and ensure continuity of care through collaboration and follow-up.

• Assist in the development of clinic schedules, recruitment, and in-service activities.

• Participate in community outreach, education events, and professional development.

• Supervise and support Mid-Level Practitioners, including record co-signing and clinical mentorship.

• Contribute to quality assurance, continuous improvement, and organizational compliance initiatives.

Qualifications:

• Education: Graduate of an accredited medical school with completion of a Family Practice, Internal Medicine, or Med/Peds residency. Board Certified or Board Eligible. Licensed in Vermont with active DEA license.

• Experience: Prior experience in primary care preferred; experience in community-based care and working with Mid-Level Practitioners is a plus.

• Skills: Strong clinical, communication, and team collaboration skills. Ability to adapt to a fast-paced, community-centered environment.

• Physical Demands: Must be able to lift up to 50 lbs, work under pressure in emergency situations, and stand/walk for prolonged periods.
