Job Title: Full-Time Traveling Physician – Skilled Nursing Facility Care

Location: Western Suburbs of Chicago IL

Position Overview:
We are seeking a full-time Physician to join our clinical team. This position is responsible for delivering comprehensive primary and acute care services exclusively within our partner skilled nursing facilities. The physician will travel locally between assigned facilities and work closely with facility staff, nursing teams, and families to ensure continuity and quality of care.

Key Responsibilities:

  • Provide on-site primary and acute care visits to patients in skilled nursing facilities.
  • Manage chronic conditions, acute illnesses, and post-hospital transitions.
  • Collaborate with facility nurses, administrators, and interdisciplinary teams.
  • Complete all required documentation, including progress notes and care plans, in a timely manner.
  • Communicate effectively with families and caregivers to update on patient status and care plans.
  • Participate in quality initiatives and contribute to reducing hospital readmissions.

Qualifications:

  • MD or DO degree from an accredited medical school.
  • Active medical license in Illinois.
  • DEA license and current CPR certification.
  • Valid driver’s license and reliable transportation.
  • Prior experience in skilled nursing, post-acute, or long-term care settings is preferred.
