Physician Assistant (PA) – Nursing Home / Skilled Nursing Facility (SNF) Primary Care

Location: Bronx, NY

Schedule: Monday – Friday

Salary: $120,000 – $140,000 + RVU Bonus Opportunities

Genesis Medical Health is seeking an experienced Physician Assistant (PA) to join our team providing primary care services to geriatric patients in a skilled nursing facility (SNF) located in the Bronx area of New York City.

This role focuses on delivering high‑quality primary care for geriatric patients in a nursing home / skilled nursing facility (SNF) setting. The provider will manage a panel of patients, coordinate care with facility staff, and ensure patients receive comprehensive and compassionate medical care.

Schedule

The provider will work Monday through Friday with flexible scheduling. Providers may choose their daily working hours (for example 8–3, 9–5, or similar). Providers must remain available and accessible by phone between 8:00 AM and 6:00 PM for patient care needs.

Compensation Structure

• Base salary between $130,000 – $150,000
• Additional RVU‑based productivity bonuses paid quarterly
• Option for fully RVU‑based compensation for higher earning potential
• Option to work as either W2 or 1099

Clinical Responsibilities

• Provide primary care services to geriatric patients in a nursing home
• Perform routine visits, follow‑ups, and chronic disease management
• Coordinate care with nursing staff and facility administration
• Participate in care management programs including CCM and RPM
• Document patient encounters in facility EMR systems (SigmaCare or PCC)

Requirements

• Active New York Physician Assistant license
• Active Medicare and Medicaid provider ID
• Experience working with geriatric patients preferred
• Skilled Nursing Facility experience preferred
• Prior experience in nursing homes or long‑term care settings is highly valued

Pay: $130,000.00 - $150,000.00 per year
