Physician Assistant (PA) – Nursing Home Primary Care (Weekend Coverage)

Location: Brooklyn, NY (Canarsie)
Schedule: Saturday & Sunday
Compensation: $500 – $600 per day (1099)

A growing medical group providing primary care services to nursing homes is seeking a Physician Assistant to provide weekend clinical coverage in a skilled nursing facility in the Canarsie neighborhood of Brooklyn.

This position is ideal for a provider who prefers weekend work and is comfortable managing geriatric patients in a long-term care environment.

Schedule

The provider will work weekends (Saturday and Sunday) and must be available between 8:00 AM and 6:00 PM , with flexibility in structuring their schedule based on patient needs.

Compensation

• $500 – $600 per day
• 1099 independent contractor
• Additional earning potential depending on patient volume

Clinical Responsibilities

• Provide primary care to nursing home residents
• Conduct routine visits and address acute medical issues
• Collaborate with nursing staff and facility teams
• Participate in CCM and RPM care management programs
• Document all patient care in facility EMR systems (SigmaCare or PCC)

Requirements

• Active New York Physician Assistant license
• Active Medicare and Medicaid provider ID
• Experience with geriatric patients preferred
• Skilled Nursing Facility experience preferred
• Ability to work independently in a nursing home setting
