Talented professional for community outreach, driving patient onboarding for remote care programs. You will represent Chronica and work with the management team to develop HRAs and Care Plans for the chronically ill patients.

Responsibilities:
- Handling the onboarding of patients into RPM and CCM programs for all practices under our purview.
- Maintaining a calendar of patients scheduled for onboarding.
- Prepare a schedule of patients using master-list provided by Chronica.
- Preparation and maintenance of accurate records and reports on the enrollment goals. Expected to enroll 4-20 patients depending on your and patients schedule.
- Communicate weekly with team using tele-features of Chronica platform.
- Provide list daily of newly onboarded patient lives and any important characteristics.
Requirements:
- Certificate in healthcare or related fields.
- Bilingual - Spanish Speaking.
- Good experience within medical delivery system.
- Outstanding written communication and verbal skills.
- Strong experience preparing reports.
- Friendly, enthusiastic, and positive attitude.
- Strong knowledge of healthcare and other basic electronic platforms.
- Detail-orientated with the ability to manage multiple projects at same time.
Note: The position is remote, PAs and other qualified medical staff are welcome to apply as long as all required certifications are valid and in order.
Pay: $40.00 - $50.00 per hour
