This Physician is a licensed Internal Medicine provider with additional fellowship training in Addiction Medicine who adheres to the highest standards of evidence-based medical practice, ethics, and professionalism. The position provides comprehensive primary care and addiction medicine services in an outpatient community health center setting, including diagnosis, treatment, prevention, and appropriate referrals. The physician is responsible for understanding clinic protocols, delivering high-quality care, and supporting integrated care models for complex medical and behavioral health needs.

This position involves delivering the full scope of outpatient Internal Medicine and Addiction Medicine services to a diverse, complex adult population, including individuals with multiple chronic diseases and substance use disorders.
Essential duties include but are not limited to:
  • Obtain detailed health histories and perform comprehensive physical examinations.
  • Diagnose medical conditions and develop patient-centered treatment plans.
  • Provide evidence-based addiction treatment, including medication-assisted treatment (MAT) with buprenorphine, naltrexone, and other approved agents.
  • Manage complex chronic diseases (e.g., heart failure, diabetes, COPD, HIV/HCV, multi-morbidity).
  • Offer harm-reduction guidance, motivational interviewing, and trauma-informed care.
  • Prescribe medications in accordance with state and federal regulations and clinical guidelines.
  • Coordinate care with behavioral health, care management, pharmacy, and recovery support teams.
  • Perform minor office-based procedures.
  • Arrange appropriate referrals for specialty or diagnostic services not provided within the health center.
  • Review and act upon incoming clinical reports (labs, imaging, consult notes, hospital summaries) in a timely manner.
  • Participate in utilization review, quality improvement, provider meetings, and multidisciplinary team conferences.
  • Assist in updating clinical protocols and implementing best practices for addiction treatment and chronic disease management.
  • Maintain accurate, complete medical records and meet all documentation, coding, and compliance requirements.
  • Provide informal consultation and clinical guidance to staff regarding addiction medicine and complex medical cases.
  • Participate in after-hours on-call coverage on a rotating basis, if applicable.
  • Perform emergency response duties if required during local emergencies or disasters.
  • Requires the ability to synthesize, hypothesize, and evaluate medical information.

  • Requires the ability to function clinically with extensive internal and external interaction.

  • Requires the ability to operate medical equipment and tools relevant to outpatient primary care and addiction medicine.

Emergency duty may be required of the incumbent that includes working in Red Cross shelters or to perform other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural. Additionally, incumbents are required to perform emergency response and management duties for Palm Beach County as required.


Education:

Doctor of Medicine or Doctor of Osteopathy degree required. Board Eligible/Board Certified in Internal Medicine required.

Fellowship training and/or board certification in Addiction Medicine required or strongly preferred.

Experience:

  • Experience treating complex medical patients in outpatient or community health settings preferred.

  • Experience working with individuals with substance use disorders strongly preferred.

  • Bilingual capability preferred, depending on population served.

  • Demonstrated ongoing professional development in Internal Medicine and Addiction Medicine.

Certification:

Health Care District Primary Care Clinics providers must be licensed and registered as a physician in the State of Florida, be Board Eligible/Board Certified in Internal medicine, Family/General practice, or Preventive Medicine/Public Health. Current BLS certification required.

Licensure:

Unrestricted license from the Florida Board of Osteopathic Medicine and the Florida Medical Board.

Registrations:

DEA registration with appropriate schedules, including authorization for MAT prescribing.

Medicare/Medicaid provider enrollment required.

Training:

Current Medicare and Medicaid provider numbers required. Demonstrates current knowledge of general/internal/family medicine ongoing professional development.
