About HEAPS Health Solutions:

HEAPS Health Solutions is a health-tech organization headquartered in Bangalore, India, with offices in Mumbai, Hyderabad, and Delhi.

We are a SaaS-based company leveraging advanced Data Analytics, Artificial Intelligence, and Machine Learning to transform healthcare delivery, strengthen care networks, and drive a Value-Based Care model.

Our core expertise lies in designing and implementing Patient Care-Coordination modules for diverse patient cohorts.

Responsibilities:

In the role of a Physician, your key responsibilities will include:

· Conduct post-discharge follow-up calls (teleconsultation) to assess patients’ recovery, track clinical progress, and ensure they are stabilising as expected.

· Identify early warning signs or red-flag symptoms that may indicate complications or potential risk of readmission; guide patients on the next appropriate steps and escalate critical cases promptly.

· Evaluate any newly reported symptoms, analyse possible causes, and provide evidence-based medical guidance within the scope of teleconsultation.

· Assess patients’ adherence to prescribed medications, laboratory investigations, diet plans, physiotherapy, and other treatment recommendations; counsel them to improve compliance where required.

· Determine the need for additional clinical services such as medication refills, diagnostic tests, dietitian support, physiotherapy, or specialist referrals, as per established guidelines (without prescribing medications).

· Provide clear and professional responses to patient queries, using standard clinical protocols and best-practice guidelines.

· Educate patients on risk-reduction measures, lifestyle modifications, medication adherence, and clinically validated self-care practices.

· Monitor deviations from the expected recovery pathway and ensure timely reporting or escalation to the clinical team.

· Document all interactions, assessments, and recommendations accurately in the medical records for continuity of care.

· Collaborate closely with the Program Manager to identify emerging clinical trends, patient behavior patterns, and operational insights, contributing to improved patient compliance and overall program effectiveness.

Skills:

· Strong clinical judgment and attention to detail, especially in assessing post-discharge recovery, symptom progression, and potential complications.

· Excellent understanding of medical terminology and post-hospitalization patient assessment protocols.

· Ability to evaluate recovery status, medication adherence, lab requirements, and identify early warning signs to prevent readmissions.

· Effective communication and coaching skills to help patients understand their discharge instructions, treatment plan, lifestyle modifications, and follow-up requirements.

· Experience in coordinating patient care through digital health platforms, especially for post-discharge follow-up. Familiarity with teleconsultation call systems or automated dialer software is an added advantage.

Requirements:

  • Strong clinical communication and analytical abilities.
  • Minimum 2-4 years of experience as a practicing physician (preferably in family medicine, internal medicine, emergency care, or community health).
  • Ability to provide structured, constructive feedback based on clinical and operational standards and handle escalations.
