Physician - Nephrologist for Sub Acute /LTC Part Time, 1099 Position EASTERN SHORE, MD

Job Summary:

Earlier discharges from medical centers require specialist supported long term care facilities to handle complex cases and our physicians provide hospital-level care, while ensuring the readmission rate remains very low, while partnering with each facility and working towards their desired clinical goals.

Our experienced and dedicated healthcare professionals are skilled in providing top-quality post-acute care, ensuring a smooth transition from hospital to home. We focus on rehabilitation, medication management, and personalized care plans to facilitate their best recovery outcomes.

We are currently seeking a skilled Nephrologist to provide services in sub-acute and long-term care settings located in Maryland.

The candidate must be an excellent communicator with outstanding interpersonal skills, including written and verbal communication and a proven ability to collaborate with other healthcare professionals to develop and implement effective patient care plans.

Must also be Board Certification/Board Eligible
