Summary

We are seeking a Physician to join our dynamic team at Colorado FirstDoc. In this role, you will play a vital part in delivering high-quality healthcare to our patients, who reside at skilled nursing and long term care facilities working collaboratively with nurse practitioners and other healthcare professionals to ensure optimal outcomes.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Collaborate with physicians and healthcare teams to provide effective treatment.
• Work with facilities to assure quality care via best practice, family meetings, and coordinating with case management and leadership.
• Maintain accurate medical records and documentation for all patient interactions.
• Educate patients and families on health management and preventive care strategies.
• Participate in health coaching initiatives to promote wellness among patients.

Requirements

• Current Medical license in the state of Colorado.
• Experience in skilled nursing or long term care setting preferred
• Excellent communication skills for effective patient and family interaction and education.
• Ability to work collaboratively within a multidisciplinary team.

Pay: $270,000.00 - $320,000.00 per year
