The Hospice physician provides remote online support to evaluate referred patients for appropriateness, and guidance to hospice nurses in the field as needed This provider is an integral member of an interdisciplinary team of nurse practitioners, nurses, social workers, certified nursing assistants, chaplain, and others in the health care team.

Essential Duties & Responsibilities:

  • The Hospice Physician provides oversight to clients and members of the interdisciplinary team to assure provision of quality palliative care including pain and symptoms management as you collaborate with the patients' attending physician.
  • Collaborates with the clients primary care physician to ensure the seamless provision and transition of care.
  • Assure appropriate evaluation, certification, and recertification of terminal prognosis at the time of admission and at the end of each certification period.
  • Promotes Hospice services to physicians and health care audiences to promote access to end-of-life care.
  • Available for phone consultation for acute patient clinical problems that cannot be resolved by the clinical team during on-call rotation. Most calls are triaged through a Nurse Practitioner prior to elevating to the physician on-call.
  • Attends weekly Interdisciplinary Team Meeting and participates in hospice care planning
  • Participates and/or leads training programs, in-service and education programs
  • Fosters process improvement within the department
  • Uses evidenced-based research to drive improvements in pharmacological and non-pharmacological interventions in end-of-life care.

Skills and Experience:

Required:

  • Doctor of medicine or osteopathy degree from an accredited medical school.
  • Experience in a medical specialty dealing with terminally ill patients, such as internal medicine, family medicine, oncology, or geriatrics and with demonstrated experience in the principles of pain and symptom management. Current and unencumbered license to practice in Wisconsin and Minnesota.
  • Hold a current DEA license

Preferred:

  • Board certification or fellowship trained in Hospice and Palliative Care preferred.

Work Environment:

Remote work with varying pace and frequent interruptions. Must be able to multi-task, think critically and prioritize well.

Required Work Schedule:

Referrals and certifications for admissions occur during normal business hours with some flexibility. 24 hour on-call rotation is required- generally 7 days on/7 days off. Almost all calls occur during normal business hours.
