PM Nurse Supervisor/ Admit Nurse

The PM RN Supervisor / Admission Nurse / Infection Preventionist is responsible for overseeing evening shift nursing operations, coordinating and completing all facility admissions, ensuring clinical compliance, and leading infection prevention and control efforts.

This role is primarily supervisory and administrative in nature, with no assigned floor patient load; however, the RN will provide direct clinical support as needed to ensure quality resident care and regulatory compliance.

The ideal candidate is a strong clinical leader who can manage staff, ensure accurate documentation, lead difficult conversations, and maintain high standards of care and infection control practices.

Essential Job Responsibilities

Admissions & Clinical Coordination

  • Complete all new admissions, including:
  • Initial nursing assessments
  • Physician order transcription and verification
  • Development and initiation of care plans
  • Coordinate with interdisciplinary team to ensure smooth admission process
  • Ensure timely and accurate documentation in the EMR
  • Communicate effectively with families, hospitals, and referral sources

PM Nursing Supervision

  • Provide leadership and oversight to PM shift nursing staff
  • Monitor staffing levels and adjust assignments as needed
  • Provide clinical guidance and support to nurses and CNAs
  • Assist on the floor when necessary to maintain resident care standards
  • Address call-ins, staffing concerns, and shift coverage needs

Quality Assurance & Compliance

  • Conduct routine audits of:
  • Clinical documentation
  • Medication administration records (MAR/TAR)
  • Care plans and assessments
  • Monitor charting for accuracy, timeliness, and compliance
  • Ensure adherence to state and federal regulations
  • Identify and address areas of risk or non-compliance

Infection Prevention & Control

  • Serve as the facility’s Infection Preventionist
  • Develop, implement, and maintain infection control policies and procedures
  • Conduct surveillance and tracking of infections within the facility
  • Lead infection control investigations and reporting
  • Educate staff on infection prevention practices and protocols
  • Ensure compliance with CDC, OSHA, and state guidelines

Staff Leadership & Communication

  • Lead and participate in difficult or corrective conversations with staff
  • Provide coaching, mentorship, and performance feedback
  • Promote a positive, professional, and accountable work environment

On-Call Responsibilities

  • Participate in an on-call rotation as assigned
  • Respond to clinical, staffing, and operational issues after hours
  • Provide guidance and support to staff during on-call periods

Qualifications

  • Current Registered Nurse (RN) license in the state of Wisconsin (or applicable state)
  • Minimum of 2–3 years of nursing experience (long-term care preferred)
  • Previous supervisory or leadership experience strongly preferred
  • Knowledge of skilled nursing facility regulations and compliance standards
  • Strong assessment, documentation, and clinical decision-making skills
  • Ability to manage multiple priorities in a fast-paced environment

  • Ability to stand, walk, bend, and lift as needed
  • Ability to assist with resident care in emergency or support situations

Pay Range: $45- $50 an hour
