Post Acute Care NP/PA for skilled nursing facility in SE Vermont

A healthcare system in SE Vermont, 30 minutes west of Keene, NH is seeking a Nurse Practitioner (NP) or Physician s Assistant (PA) to join their team and provide care to residents of their affiliated Skilled Nursing Facilities.

• Board Certified Nurse Practitioner (NP) or Physician Assistant (PA)
• Full time; M-F 8-5p
• Provide care and essential medical services across multiple Skilled Nursing Facilities, hospital affiliated
• Prescribes modalities including but not limited to medication, nutrition, exercise, and traditional nursing treatments.
• 2 years of experience in post-acute care/SNF's highly preferred
• Working collaboratively with Family Practice Physician
• DEA License
