Responsible for providing early intervention and prevention services to strengthen protective factors and reduce risk factors. To provide trauma-informed care and support social emotional development in infant, toddler and preschool age children and their families. The Prevention and Early Intervention Coordinator (PEIC) will make care visible through coordinating with schools to increase access to care, assist with removing barriers to care, promote follow through with care by providing the linkage from identifying the need to engaging in the service for increased patient navigation, promote collaborative and coordinated care. The PEIC will administer screenings, conduct observations, consultation and coaching, training, conduct groups, and make referrals for services. Provide resource referrals to increase access to equitable, trauma-informed, strengths-based, social emotional learning and racial equity-centered, multi-tiered mental and behavioral health services and training.
Primary Functions:
- Coordinator with school personnel and conduct a needs assessment.
- Conduct individual and classroom observations.
- Conduct screenings and assessments in a timely manner, create treatment plans and individualized behavior and support plans for students.
- Review and share results of screenings and assessments with identified school personnel and families, conducting follow up on any identified concerns.
- Provide training and technical assistance to school personnel, families, and community partners as needed.
- Coordinate, coach and consult with school personnel to promote best practices for social-emotional development and increase school personnel’s ability to increase connection with students, increase student’s social-emotional and communication skills for increased self-regulation.
- Participate in treatment team meetings
- Coordinate and provide evidence-based, trauma-informed, social emotional learning (SEL)-grounded, strengths-focused and equity and racial equity-centered suicide prevention, intervention, and postvention groups.
- Coordinate and provide evidence-based, trauma-informed, social emotional learning (SEL)-grounded, strengths-focused and equity and racial equity-centered suicide prevention, intervention, and postvention training.
- Prepare and maintain documentation according to the Department of Mental Health, Swope Health Services and HRSA policies. Including but not limited to all case record documentation, treatment plans, and other recordkeeping as needed, so that records clearly reflect actions from referral to service termination.
- Establish and build working relationships with individuals, school support teams and clinicians, families and other organizations in the community in order to form partnerships that contribute to comprehensive delivery of services.
- Perform other duties as assigned.
Experience & Qualifications:
- Minimum of one year of experience working with infants, toddlers and preschool aged children and their families.
- Possess knowledge of infant/early childhood development.
- Ability to build and maintain relationships with children and their families as well as school personnel and community partners.
- Experience with creating individualized behavior and support plans as well as creating treatment plans.
- Possess excellent communication skills.
- Ability to train and provide technical support.
- Working knowledge of community resources.
- Possess the ability to work independently with minimal supervision.
- Knowledge of Trauma-Informed and Strength-Based Practices.
- Awareness and understanding of cultural of clients, staff and other community partners.
Educational Requirements
- Master’s Degree in Social Work, Counseling, Psychology or related field.
Physical Demands - General Activities:
- Must be able to lift up to 15 pounds at times.
- Must be able to push up to 15 pounds at times.
- Prolonged periods of sitting at a desk and working on a computer.
