We are expanding and seeking highly motivated Nurse Practitioners to join our growing team.

The Nurse Practitioner will provide comprehensive, evidence-based medical care in home-based and facility-based settings. This is an autonomous role ideal for providers who are confident in independent clinical decision-making and committed to high-quality documentation and CMS-compliant practice.

• Perform comprehensive assessments and develop treatment plans
• Diagnose and manage acute and chronic conditions
• Provide wound debridement and advanced wound management (if applicable)
• Manage chronic pain treatment plans (non-opioid preferred model)
• Evaluate cardiovascular and vascular conditions
• Coordinate home health, DME, labs, and specialty referrals
• Complete Face-to-Face encounters for home health certification
• Ensure CMS-compliant documentation and coding
• Participate in quality and compliance initiatives

Job Types: Full-time, Part-time, Contract

Pay: $90.00 - $110.00 per hour
