Responsibilities
- Review and analyze medical records, clinical documentation, and encounter notes to assign appropriate ICD-9, ICD-10 diagnosis codes and CPT (Current Procedural Terminology) procedure codes.
- Ensure accurate DRG (Diagnosis-Related Group) assignment for hospital outpatient services when applicable.
- Apply correct medical coding principles to support precise billing and reimbursement processes.
- Collaborate with healthcare providers and billing teams to clarify documentation discrepancies or ambiguities.
- Maintain compliance with all coding guidelines, payer policies, and regulatory requirements.
- Update and manage electronic health record (EHR) or electronic medical record (EMR) systems with coded data efficiently.
- Assist in audits by providing detailed documentation of coded entries and supporting data integrity efforts.
Requirements
- Proven experience in medical coding within a primary care or outpatient setting, with familiarity in DRG, CPT coding, ICD-9, ICD-10, and ICD coding standards.
- Strong knowledge of medical terminology, anatomy, physiology, and medical records management.
- Hands-on experience with EMR/EHR systems used in outpatient clinics or medical offices.
- Excellent understanding of medical billing processes and medical collection procedures.
- Ability to interpret clinical documentation accurately and apply appropriate codes consistently.
- Prior experience in medical office environments is highly preferred.
