Primary Care Nurse Practitioner

Overview: At Avalon Medical Center, we are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive care to patients, with a focus on preventive health measures, diagnosing and managing acute and chronic illnesses. This role requires a strong commitment to patient education and the ability to work collaboratively within a multidisciplinary team to ensure optimal patient outcomes.

Key Responsibilities:

• Conduct thorough physical examinations and assess patients' health status.
• Order and interpret diagnostic tests, including vital signs monitoring.
• Develop and implement individualized care plans based on patient needs and preferences.
• Provide family planning services, counseling, and education to patients.
• Utilize medical terminology effectively in documentation and communication with the healthcare team.
• Ensure adherence to established standards of care and practice guidelines.
• Perform clinical procedures while employing aseptic techniques to ensure patient safety.
• Collaborate with healthcare providers to coordinate care, share insights, and develop treatment plans.
• Educate patients on disease prevention, wellness practices, and self-management strategies.

Required Skills & Qualifications:

• Advanced clinical knowledge in primary care settings, with proficiency in assessing and managing common health conditions.
• Strong knowledge of medical terminology and documentation practices relevant to primary care.
• Ability to perform key tasks such as accurately taking vital signs, interpreting lab results, and developing treatment plans.
• Experience working with patients who have developmental disabilities is highly desirable.
• Excellent communication skills to engage patients in discussions about health education, lifestyle changes, and preventive care.
• Strong organizational skills to manage multiple patient cases efficiently and effectively.
• Ability to collaborate with multidisciplinary teams to deliver comprehensive patient care.
• A strong sense of empathy, professionalism, and dedication to improving patient health outcomes.

Education and Experience:

• Master's degree in Nursing with a specialization in Family Nurse Practitioner (FNP) or Primary Care.
• Current and valid Nurse Practitioner license in the state of NY.
• years of experience in primary care or a related healthcare setting preferred.
